How to get referred

If you require transport support or pre or post bereavement support you can refer directly to us. Please call 01434 600388 for transport and 01434 600384 for family support.

For nursing care, we take referrals from a member of the Primary Care team eg. District Nurse, GP, MacMillan Nurse or Community Matron. For information on our nursing care please call 01434 604321.

Job Title: Registered Nurse (Zero Hours Worker) and Hospice Support Worker (Zero Hours Worker)

Location: Community of Tynedale, Scots Gap and Ponteland


Being diagnosed with a life limiting illness can be a daunting experience, and for many people, being cared for in their own home can make a big difference.  We are looking for motivated registered nurses and hospice support workers who are committed to delivering high quality end of life care. You will be employed on a zero hours contract. We are especially keen to recruit staff for night duty and weekends. Experience in community work, palliative and end of life care is desirable but not essential. These posts would be ideal for staff who wish to choose flexible working patterns.
