Our Catchment & Eligibility
People registered with a GP in our West Northumberland catchment may be eligible to access our care services.
Our catchment covers residents registered at one of 16 GPs in Tynedale, Ponteland and West Northumberland. We cover an area of over 800 sq miles, often remote locations where people face long journeys to hospitals.
Tynedale Hospice at Home operates four services with an emphasis on holistic care and keeping families and communities together. Our nursing care is home-based. We don’t operate a bedded Hospice. Please see below a list of GPs in the catchment area and a table outlining eligibility for each service.
GP Practices
Patients must be registered at one of the following GPs to access our care services. Make sure your GP is listed if you are accessing services for yourself, or check with the patient if you are referring on behalf of someone else.
